Hey, I'm a physio from Belfast, Northern Ireland. I've previously participated in GPP (and loved it) but like doing my own programming too much to stick with it. I love all forms of exercise and try to build encourage a positive experience of movement and exercise with my patients/clients so they can get out of pain, feel good and maximise fitness qualities for their hobbies or sports. I previously did a mentorship with Pat Davidson on creating our own model around exercise and I took it one step further and developed my own physiotherapy and S&C model which I've found to be unbelievably valuable in creating a deeper understanding and applying this to clients (and myself) I just love what I do and am happy to offer advice / help to anyone who wants it!

Patella Tendinopathy - Adapting Training
Hi all Apologies in advance if this isn't the type of question to ask here, but I guess it fits in with my current training and is a pretty sports specific issue, so may help others here. I've been struggling with what I believe is Patella Tendinopathy for approx six months. I've followed Jake Tuura's recommendations (I bought his program) and although at times it seemed to relieve the pain, I'm still at a stage where I can't engage my right quad (literally cannot tense it) and as soon as I go anywhere near parallel, there is just no strength there and my knee just fails and I have to bail out of the movement. Side info: I have Rheumatoid Arthritis, and go in for infusions every year to be able to keep going. Last year was the first time in 10 years (diagnosed at 27) that I felt well enough to start playing basketball again, so I joined a local team and had an amazing first season. Come the end of the season, I've ended up with this problem, so I'm currently taking time off in pre-season, hoping to resolve it for starting the season again in September. Has anybody gone through a similar issue and managed to resolve it? I run a pretty simple leg program that involves the isometric recommendations for PT, along with the usual squat variations, hamstring curls, calf, glutes. Look forward to reading any replies. Hey @Seb Atkin, I've some experience with rheumatoid arthritis and tendonopathies... because the arthritis is an auto-immune disease and so it could affect your bodies' ability to recover as effectively from the physical stress of training and it can have an inflammatory effect which could also have a knock-on effect to the tendon. Just some general advise that may be useful. Are the recommendations on isometrics the standard 3x45sec. on a leg extension machine? I'd look at trying to progress these, whether it's longer durations or higher load (might take some experimentation to see what works for you) in an attempt to create tendon adaptations. For anyone with long standing tendonopathies I usually keep the isometrics in year round, they can be modified or changed with different exercise selection to help reduce monotony. Lots of statice quad contractions would be useful too to help reduce quad inhibition which would be present when pain is also there. I agree with @Angus Bradley on the E stim machine, might be a worthwhile investment to help get those max voluntary contractions back. I'd also suggest looking into David Grey Rehab, He's lots of great content on his social media that would be useful to look into with regards to lower limb coordination and cocontractions.
