Here's Alex's exact questionnaire template he uses to get testimonials from his clients.
He calls it "Success Interviews" and outlines the "Epiphany Bridge" to create "Lifecycle Ads" for a 60-90 second video you can use for content, bonuses and ads.
PHASE 1: BEFORE PURCHASE
Q1: [INTERNAL] What was life like before internally - before you saw this, did this?
Q2: [EXTERNAL] What were the external stats other people would observe about you before you decided to do this thing?
PHASE 2: DURING PURCHASE
Q3: [EXTERNAL] What made you actually take the next step, externally?
External - I was making $3k/mo, behind on payments, almost lost house, had to downgrade our car (external stuff)
Q4: [INTERNAL] And internally, what were the things you were most concerned about?**
Internal - I was struggling, my wife didn’t respect me anymore bc I couldn’t make money

Then I saw an ad from Alex and it kinda caught my eye.
Q5: What were your biggest concerns?
I didn’t know if it was legit
Q6: Well what made you decide to do it anyways?
Well I saw the other people that had testimonials and saw a guy who had almost the exact same model as me… so if it worked for that guy maybe I’ll give it a shot.
PHASE 3: AFTER PURCHASE
Q7: What’s life like now, externally and internally?
Now I was able to get back the car, I upgraded, we moved to a better neighborhood, my kids can go to a better school and I feel better about myself.
OUTCOME: Condense into 60-90 sec video and also becomes an ad.
