HELP! What would you do?
I had someone reach out to me about something that I have found happening all over the world. The message said: The COTA and I have been belittled by two new grad (one year out) PT and PTA who went to the recent APTA conference in Boston. Rumors I'm hearing at they want to start a HIGT program because me and the COTA aren't already doing it, they are introducing it to our manager even though I talked with the manager before I took the Walk the Walk Course to hopefully have my employer pay for the course. Surprise! They didnt'! I also approached the same PT about starting the HIGT program together after an inservice by Dr. Hornby (PT). They have ignored my ideas on how to increase intensity through focusing on the various subcomponents of gait. What it feels like is there is no respect for OTs at my organization. It's deflating. It's frustrating. I don't know what to do anymore to work cohesively with the PTs when trying to collaborate and getting shut down repeatedly and being disrespected. Lets discuss a few things: 1. Does anyone else have similar situations happening to them? 2. What ideas do you have about this particular situation and how would you go about addressing it? 3. How can OT's begin to get more respect in the medical setting? 4. Anything you would like to say to this OT who is feeling down and struggling?

This is a hard situation to be in. I have had my own experiences where I have been questioned for implementing HIT within my practice. I agree with Sharon’s comment, if going to your manager again with the research and the practical examples/ideas of how you can increase intensity via addressing the subcomponents of gait within occupation, then just do it and collect the data. I was fortunate that my manager supported me in approving the walk the walk course, but I definitely get a lot of resistance from some (definitely not all) of the physios I work with. I get pushback , I feel frustrated with being questioned, PTs and OTs (for that matter) talk behind my back about what I am doing - thinking I am going beyond my scope of practice because I do stairs, I have my clients walk and run. What I can say is this - keep pushing yourself and those boundaries, show what our profession is truly about, maximizing performance through occupation. We have all the skills to push our clients and to do high intensity training, and they won’t be able to deny your clients gains.

Montreal, Canada I have been practicing for 17 years. Initially I worked in geriatrics (mainly neuro, stroke, and some ortho) in an inpatient setting. Then for the last 11 years I shift to just inpatient stroke/neuro rehab and now I am in outpatient stroke and neuro rehab. I love working with stroke/neuro clients!!
