Salem, 19yo, med student, I need advice
I am turning 20 this December. I've always been a brilliant student, I've always been what people called lucky and I didn't struggle with sled doubt or motivation. When was I 17yo I took the national exam which was necessary to get to uni and med school, it wasn't tough for me to get med school level grades, what I aimed for is to sacrifice an entire year and become the best in the entire country. I worked like a mad man with every ounce of intelligence and dedication I had, at the end the result was underwhelming, I was in 0.1% but far from the best, I felt robbed of something I was the only one willing to sacrifice enough for. There's was no redoing, I got chucked into med school, I was confused and didn't know what to do pretty much, my self doubt destroyed me, and physical laziness and weakness made me an absolute disgusted of myself, I barely passed the year while people I was far ahead of were doing better than me I was disgusted by myself. Second year, I recovered, I got into bodybuilding (which I became obsessed with, I'm now making a lot of progress) and studied better and ranked better, but still I self sabotaged enough to reach my full potential, there are nights in which I want to kill myself for the guilt I feel for not trying as hard as I know I could be. I am not happy with my life in third year, it's the same pattern repeating itself. I used to think I'm strong mentally but I'm fragile, I admit it, I have self doubt, people who I used to be better than I look at them now as unbeatable rivals. I hate myself, I also try to build skills which I can eventually monetize in the online space but bad time management makes the progress stall. I don't know what to believe in as fact. I need you to tell me, what mindset shift do I need? What do I tell myself? How do I just stop the self sabotaging behavior and put my all in once again.

Hey Salem, first off, I respect the honesty you're bringing here. A lot of guys go through similar battles but don’t have the guts to admit it. You’ve already accomplished a lot, but I understand you’re feeling like it’s not enough. Here’s the thing, you’re measuring yourself by external success, but real growth has to come from within. Your worth doesn’t depend on being the best or outcompeting others. Your real value comes from being created in God’s image, seeking your purpose, and becoming the best version of yourself. Focus on who you’re becoming in the process, not just the outcome. Now to address your questions: Mindset shift: You need to shift from obsessing over being perfect to embracing progress. You’re not going to get everything right every time, and that’s okay. Romans 5:3-4 reminds us that challenges build endurance and shape our character. God is using your struggles to refine you, so instead of seeing this as a defeat, view it as a chance to grow. You’ve been disciplined before (top 0.1% and killing it in the gym!), so it’s clear you have the ability. Start seeing setbacks as part of the journey, not as failures. What to tell yourself: Remind yourself daily that your worth isn’t tied to outperforming others—it’s tied to your effort, character, and relationship with God. Tell yourself that you’re in a process of growth, and as long as you’re taking consistent action, you’re on the right path. Say things like, “I am a work in progress, and I am proud of the effort I’m putting in.” Focus on small victories, not just big achievements. Stopping self-sabotage: Self-sabotage often comes from setting expectations too high and then feeling overwhelmed. You’re juggling med school, the gym, and trying to build online skills—that’s a lot! Instead of trying to be perfect in all of these areas, break your goals down into small, daily steps. Proverbs 13:4 talks about the power of diligence—work on improving 1% every day. Discipline yourself in the same way you have in the gym. Don’t demand perfection, just consistency.
