B - proximal LHB. Overhead punches overload pretty much all of the structures listed in the options. However based on the subjective finding of pain location, and that the LHB being compressed / concentrically involved during the aggravating movements. The other options may be aggravated during the movements listed, however LHB is most likely to be injured first during an overhead punch as the tendon is forced to transmit force (antagonist during punch) in a compressed position.

Welcome to the FIRST case study in Case study corner! This week, I have a case study that might just bring you to your KNEES! CASE STUDY: Tim is a 27-year-old semi-professional soccer player. One week ago, during soccer training, Tim felt pain in his left knee after kicking the ball. Tim was able to keep playing. He reports that the pain has been increasing in intensity over the last few days especially with activities that require full weightbearing and/ or full knee flexion. WHAT WOULD YOU DO NEXT?

Based on previous comments, the main thing I'd do is palpate the medical knee, as I'm suspecting a compressive force on the medial knee during that kick across the body. Perhaps this has aggravated either the ligaments or tendons attaching at those areas.
