Thailand - any malaria or dengue concerns?

Internet sources state most of Thailand is "low risk", just wondering - what's the actual situation on the ground there?

Anybody taking anti malarial and if so what are the issues? Cost / side effects etc?

Any other health issues that are less well discussed / understood / good to be aware of?
