on 3t scanners with fast acquisition we can sometimes see these hypoperfusions, especially in shoulder MRI after Gadolinium. nothing is seen on PD T2 STIR etc, only post KM, indication higher pressure on tissue from positioning, coil padding or even subacromial space (might indicate risk of subacromial impingement (just an idea, no evidence).
