How Does PO3 F@(K Up our Valid Entries

I'm having a hard time understanding how PO3 messes up valid entries. You get a valid entry, you've checked everything including FMZ hunt but BOOM! PO3 comes from nowhere and says, "Gotcha!"

Really frustrating.
